Yizkor List: If you wish to add names to the Yizkor list, please contact Sara in the office or fill out this form.  If you have previously added names to the Yizkor list, you do not need to add them again.  Traditionally, a donation is still given which can be done at the bottom of this form.  If you have new people to add, please do so by using the same form.

Names need to be submitted by 4pm on Wednesday, September 19 to be included in the Yizkor service.
